    Why does my form keep collapsing and erasing data?

    I have a sign in for that is used for players in my free poker league. Each day we sign them in and to make the form less cluttered for my directors who run the games, I only have the next field of player name appear once data has been entered into the previous name column. It worked fine for a month, but lately the fields are getting erased or at least hidden and do not transmit completely. This is making us revert back to paper logs as a backup. I have the form set to auto save often and save for 10 hours so I don't know why it is doing this. I do not want to find another service cause it took a long time to create this form. The form I am talking about is called Regular Game E-Form Player Log. Please advise.

    I did a test in a clone of your form. But I'm able to get all the information I entered via email.

    One think I noticed is that you got a lot of conditional for Player's names. Rather than that you could use a dynamic widget so your users keep adding as many players as they want, such as:
