Sign up Form, add another option

We have sign up form for Chirstina Meditation and Centering Prayer - I need to add another option   please help

You can edit that form in the form builder so you can add options. Go to your My Forms page, select and edit the form. Is this what you mean about your question? What exactly do you mean with add another option? We'd appreciate if you can add more details.
