How to create the combination 'tick box' + 'amount' ?

    It is not clear to me how to create the following form item: customers can combine food items (breakfast delivery service) and choose the number of each, eg.

    Tick boxes       Number dropdown

     ↓                                   ↓
    (x) item 1 - amount= (1)
    ( ) item 2 - amount= (0)
    (x) item 3 - amount= (3)
    (x) item 4 - amount= (3)
    ( ) item 5 - amount= (0)
    (x) item 6 - amount= (2)
    (x) item 7 - amount= (6)

    etc.

    We can do this with a Purchase Order tool from the form Payment Tools. It is actually not connected with any online processing systems, so you can use it without online payments.

    To put the quantity on the same line as the product, I used a CSS from the following thread:

    If the product Check Box is not mandatory, you might consider using a Quantity Gift Registry widget.
