    The form works perfectly on my website but I want it to go through Jotform.
    I tried cloning the form into Jotform, but all I get is a garbled repeat of the dates etc.

    Please look at  Http://www.christmaslights.co.za/form.html and advise me.

    Why don't you just create a new form on JotForm form builder and use that? That would be easier than trying to edit this existing code. It can also look nicer since JotForm has a date picker where user can click on a small calendar icon to choose a date.

    If you still would like to use this form, you must still create a form in JotForm and make sure that your form has all input fields with the same names and the form should point to jotform.com site. Take a look at the source code of the form you created on JotForm to see field names and where the form should submit.
