    Hi,

I am looking for a solution to create forms that can be embedded to our website and can also be used in payments collections. Jotforms seem to do the job, but i have the following requirement:

When a new company signs up for my services i need to send out a form to collect information about all the users in the company.  How can i accomplish that with one form that will be filled out by one person?

    Concerns:

    - Data for each contact should come in as separate entries ( so they can be imported to our CRM etc)

    - Should be tied to that specific client

    - Ability to be dynamic as the number of users changes according to the clients company size

I would appreciate some guidelines or ideas...

    Thank you

    Hi Argyris

How about if you use the Infinite List Widget on your form.

This widget will allow you to add multiple input list within the form.

Here is also a demo form  http://form.jotformpro.com/form/41873880623966? that uses this widget

    - Ability to be dynamic as the number of users changes according to the clients company size

Can you elaborate more what dynamic you were referring to here.

Hope this help. Inform us if you need more assistance.

    Thanks.
