Domain Name ".US" & ".CA"

How do I change the Jotform domain name from .ca to .us......or change both to jotform.com/(formname)??

Hi,

Those are just the different domains and servers that can be connected to.  Simply changing the domain name will change the server you connect to.  For example, the form you linked resolves to the .ca extension.

http://www.jotform.ca/CGCDallas/InitialRegistration

Some JotForms will not be available from certain extensions though.  So even though jotform.us is one of our servers, your form is not available using that domain.  However,

http://www.jotform.com/CGCDallas/InitialRegistration

does work.  Just change the domain name and you should be all set.
