Hello,
When a customer completes a form on my site through your service, I have it set to email me. For some reason, one of the fields does not show up it the email and it's an important one. See attached pic. The "Wedding Date" field is always blank, however, all the other fields show up.
Please advise.

Hello Joe,
I did check your form's notification email alert template and found that the field name of the "Wedding Date field is not correct. Please check the screenshot below:
This happens when you edit your email alert. When you edit your email alert, it becomes dirty and after that any changes you make in your form will not reflect in your email alert. You will have to manually update it as shown in the screenshot above.
To solve your problem, you should delete your existing email alert and add a new one. A new email alert will always have all the form field in your email template. The following guides should help you:

Add new email alert: http://www.jotform.com/help/25-Setting-Up-Email-Notifications

Hey Joe,
Sorry to hear that you are experiencing this issue. Based on your screenshot, the data field name for "Wedding Date" might have been mistakenly changed or deleted. This is not going to be a difficult problem to resolve. Just follow these instructions and you will be fine.
Please follow the images below.
Save and test the form. Please view the image below for my outcome after the adjustments.
This should have resolved your issue. Please let me know if the issue still persist.
