I do not receive an email notification of the form when a new one is filled out.

    The purchasing / asset transfer form,

    1. doesn't request the approval how it was originally set up to

    2. doesn't send the form to purchasing in the correct format

    1. I cannot find any form associated with your email, that's why you might not be receiving any email.

    2 if you have created a form using another email address, please let us know what is this.

    Also, I want you to  be aware that we just had a temporary problem with our default email sending method so we switched to a backup method, the problem should be soon resolved.
