    Import PDF Form to Jotform

Asked by miccol3926 on September 17, 2014 at 09:15 AM

I have a Patient intake form that is in .pdf.  Am I able to import this file? Or what filename must it be saved as?

I would like to convert this form into html so that I can imbed it on my site.

Where is the patient information saved and will it be encrypted? Thanks

Unfortunately, we do not support importing from any local documents like PDF. Jotform system isn't capable to do so. You may consider creating the form from scratch or find available templates in the form templates page http://www.jotform.com/form-templates/ that is similar to your form so you will only do minor edits.

Your other questions will be answered on a separate thread. I have split it as it doesn't seem to relate with your main question. http://www.jotform.com/answers/430159

    Thank you! :)

I am truly sorry, I just found a Jotform App that is capable of importing fillable PDF form into a Jotform form. Check this out: http://www.jotform.com/spread2jot

I am not sure if this app is on beta release or something but if you encounter any problems, feel free to inform us here.

    Thank you!

    Great news!

Your can now import your fillable PDF form to Jotform.

    Here's the import page: https://www.jotform.com/adobe-formscentral-alternative

When you click the Import Forms button, select the Upload PDF and upload your fillable PDF file. You will be informed via email once your form is ready.

I do not have the Upload PDF as an option. After I click on the Import Forms button, I only have two options, Import Form from a web page or Import Form Clone existing form.
