My JotForm is disabled :( Please help

    Your form and account were suspended by our automatic phishing detector.  Your form title "Forgot Your Password?" is what set it off.  Asking for any information related to passwords is against our terms of use and simply using the term in your form can occasionally set off our system.

    I removed the form and reactivated your account and forms.  You should be able to use them as normal once again.
