To the best of my knowledge, my forms are setup correctly. The desired effect I'm trying to achieve is that when a customer submits an inquiry on any one of our forms, their email address is automatically used for the "Reply To" field in Gmail. It was working as recently as yesterday, though seems broken today and no changes in configuration have been made on my end to trigger this issue. As it currently stands (broken) the "reply to" field is automatically populated with firstname.lastname@example.org. Help!

Sorry to say but, Jotform is currently experiencing some issues with their email system. I believe this is the reason why you're experiencing this problem at this time. They are currently working on fixing this issue and they are now using their backup system to deliver emails.
I hope that the problem will be rectified soon so it can ease your workload. Until then please give them some time to get this issue resolved.

Yes, seanmccthy is right. We are experiencing some issues with our default email sending method and I think this problem is related to that. As mentioned, this is just temporary and our developers are currently working on fixing it.
I am also able to replicate the problem:
I have now forwarded this thread to our developers so you'll get an update once this problem is fixed. We advised some others to use the following alternatives:
SMTP Method from Mandrill: https://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address
SMTP Method from your hosting provider: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form
We thank you for your understanding on this matter.
