How to show form headers and text data on submissions page and PDFs

I've been checking out some of the forms I've designed and I'm baffled why some information is missing and why I can't view the whole form as you can in "preview", but with the completed information in the boxes.    The point in why I joined Jotform is to be able to view all questions/answers from the submitter and for them to be able to keep a PDF copy of each form for their records too (and print if required - I did add the print button at the end of the forms).

Most of my forms have certain information on them for the client to read but they don't have to acknowledge; a sort of statement if you will, advising them of what to complete and what not to complete.  However, when I receive the submissions, the information is missing from the viewable form.    Please check out the link to one of the forms below and you'll see what I mean.

The paragraph starting " I hereby give permission" is not showing on the printable/submitted form; neither is "Walk the Line's resident pets".  I'd like to view the whole form.  Can this be done?

    Thank you for contacting us.

Let me please inform you, that there is a 'Show Headers and Texts' option available. Please go to your form Submissions page, click on a gear icon, and enable the 'Show Headers and Texts' option. It will include the form headers and text data to the submission preview and generated PDFs.

Also, we have a submission PDF download link option. The PDF link can be added to your form Thank You Page, Email Notification and/or Email Autoresponder.

Thank you very much.  How do I install the PDF download link option?

I've just worked this out on the "thank you" message.  Now, can I (not the customer) view the PDF link to see all information for the submitted documents?  I've tried this out, by completing one of my own forms and sending it to myself, and the PDF version with all the submitters information as well as the questions, is what I'd like to view - can this be done?  Hope this makes sense!

    Thanks
