We've been using your service for about a year. We have a few application forms on our website that send email alerts to both us and the applicant. Today we've had two complaints that the email to the applicant isn't being sent on two different application forms - http://www.jotform.us/form/30665506871155 & http://form.jotform.us/form/30724337017145. I am also unable to receive the email after filling out either form however we do receive the notification email still. The "test email" button in the compose email window does send the broken email.

Problem solved, we changed our email password and it messed up the SMTP settings.
