No luck with your FREE form set up. Tried to save my work each time and it dumps out. I wasted two hours on trying to make this work.

I need a form that my district managers can use when they go in a store and meet with managers. I want the time documented, subject, who they spoke to, what was the weather like, guest flow, etc.  I have several parameters I wish to monitor. I tried your tracking student progress template and it seemed great until I couldn't get the drop down to allow more than one area of discussion chosen.

Also...we don't have a protected website to embed this to. Where would I put the form once it's done. I want to simplify their life in the field with an easy access online form that takes minutes to fill out and gives me a record of their visit in the store. It tells me what they're doing towards their goals and how the store is doing.

I have a CTuit dashboard site. I'm wondering if this could be a link on that site and available to them.

Your idea is great but I just need to make it work for us.

What you are looking for can be accomplished with a Widget called Checkbox in Dropdown, which will allow you to select more than one option in the dropdown list.

That's not a problem you can share the direct link of the form with your co-workers. However, if you already have a website, you can create a webpage and embed into it.
