We have been trying to get some revisions to our account, and this is our last resort. Please see below:
Please see attached samples of receipts, which we have noted changes to be made.
Our E-Mail should appear as:
email@example.com., and any phone number that is provided on receipts should be 434-239-9278
We also would like to make an additional change to our JotForm that shows up on our website, by deleting the account number option....it confuses people that don't know their account number, and it doesn't even reference the account number on the confirmations that I get.

Our records currently indicate you have a guest account associated with the email address "firstname.lastname@example.org". You need only register the account to make the changes you would like. If you are trying to change an already registered account, you can change your email address from Profile section of My Account page. To do this, open My Account page, on the Profile tab, you will see email field. Click Edit on the right side of the email field.
