Photo Element will not load in forms!

Photo element is not working.  I've added it to my form several times and it won't load.  It use to load a month ago, now it doesn't work.  Can you help me?

I am sorry but I am not able to identify which Photo element you were referring to.

Was it the Take Photo Widget http://widgets.jotform.com/widget/take_photo ?

I checked your jotform on the website http://www.polarbearpuppies.com/PuppyApplication.html but I did not find any photo or image widget on it.

Please clarify so that we can properly check.
