To achieve your requirement, you should add "Appointment Slots" widget in your JotForm. This widget will allow you to book appointment form the available schedule.
To send an auto-responder email to the user/client, you should actually add an auto-responder email to your JotForm which will send submission emails to the user. The following guide should help you: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder
