i want to create a an appointment form with an auto responder which reply to the clients automatically that they have successfully requested for the appointment.

can anyone help me plz ?

Hello ALISHER,

To achieve your requirement, you should add "Appointment Slots" widget in your JotForm. This widget will allow you to book appointment form the available schedule.

To send an auto-responder email to the user/client, you should actually add an auto-responder email to your Jotform which will send submission emails to the user. The following guide should help you: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder
