I see that you are using First Name and Last Name separately on the email notification. Since we have used Full Name field in the form, the firstname and lastname field will not be recognized resulting blanks for these fields in the email.
I've changed this FirstName and LastName to Full Name in the email notification. You should be now receiving the complete name.
