    Hi regent_vincent,

Thanks for dropping us a line. However, I'm not quite sure what you meant by questionnaire A and B. Are they two distinct forms in your account? I tried looking for them in your My Forms List but did not find them. Kindly advise so that we can further assist you.

Thank you for your instant response. I previously added INDIVIDUAL onto my website, and just now I've developed another questionnaire named PRE ASSESSMENT, and would like to replace the INDIVIDUAL one...~

    Hi regent_vincent,

In order for us to provide you the most suitable solution for your issue, could you provide us the URL of the website where the form, entitled "Individual", is currently embedded to?

Your response will be greatly appreciated. Thank you.

    Hi there,

You can change the form Individual in your website by replacing its embed code with the newly created form (Pre-Assessment).

You can obtain the form's embed code by clicking directly on "Source Code" from the "Setup & Embed" tab or "Embed Form" and select an embedding method.

Hope this helps. If you do need further assistance, please feel free to contact us.
