Is your problem solved? Are you not able to see any specific form in your account or you are not seeing any forms at all. I did check your account and did not find any issue with that. You seems to have 2 forms in your account.
If you are not able to see any forms, I would suggest you to clear your browser cache and see if that solves your problem.
