I was wondering if including the uploaded images in a PDF of the form was ever added or made available as an option for a form as the last post was almost 2 years ago?  This would be extremely helpful for my business.

This feature is already added I just made some test and it shows the image you can view the sample here https://shots.jotform.com/jed/img/others/287367980570469221.pdf.

If you are having problem seeing the image can you please share the form URL to us or if the form is currently embedded to a website please share the link.

Thanks for that.  However, I am not necessarily needing to see an image that someone who filled out a form uploads as an attachment.  My question is in reference to an image that I have on the form itself.  When creating a form, under "Form Tools", there is an option to upload an image to my form, like a logo.  When that form is converted into a PDF, my logo is no longer there as part of the form.

Is there a way in which I can show my logo like how we can show headers and texts?

    Hi,

If I understood you correctly then these screenshots would be what you are looking for:

View in submissions before getting the PDF

Now to see the image all you need is to click on the gearwheel on the right and select to have the fields and headers shown to you.

Do let us know if this is what you were looking to achieve and especially if it is not.

    Best Regards,
