I can't get the form to work correctly. The City and State fields do not work. I have tried deleting these fields and creating new input boxes for City and State. And it still doesn't work correctly. I even tried using a default premade membership form and even that one would not work correctly. It did not send the name and city.
I'm using the preview form inside jotform to trouble shoot the form.

I've fixed it for you by recreating your form notification email. There are fields which are not present in the form notification email body.
We have a system update tool that works in the form notification email every time you'll made some changes to your form. But this tool will only work for the default format of the notification email. If you modify the body part, especially the table or the fields, this auto update feature will become inactive. When it's inactive, it cannot update your form notification email causing this issue with missing field or field data.
