    In the search ID section of the PHP script, I assume the 'table_name' has to be updated to my table name, is that correct?

    Yes that is correct. You have to replace it with the name of your table in your MySQL database. In the sample below I use my table name "postsample" to connect to my MySQL table.

    $query = "SELECT * FROM `postsample` WHERE `submission_id` = '$submission_id'";
