One of the reasons why you are unable to access or log in to your account is you might be registered for a guest account. As far as I know, this type of account is created when you first build your forms without signing up to JotForm.

In order for you to access your account and forms again, I would suggest you to sign up for a free account using the email address that you are using when you first created your forms.
