    Hi Adam,

    I checked the embed codes and couldn't find anything wrong with it. I think the problem is with the CMS you're using for your website and has nothing to do with the form itself . If you look closely, the header "Life Support Enrollment Form" is actually not a part of the form, yet it doesn't show up on IE either.

    Upon looking at your site codes, it appears that that header is inside a div container together with the form codes. That means that under IE, that particular div container is not displayed.

    With that in mind, I suggest that you contact your website's administrator for further assistance as this is a little out of our jurisdiction. Thanks!
