I've recreated API keys, cloned the forms and done anything else I could find in the forums but the info is not being sent. Not sure what's going on.Page URL:

Please make sure that you have added a Date field to your Mailchimp list before integrating, to do this go to Settings > List Fields.
Then add the date field :
Then proceed to integrate the form with your MC list , this time it should map:
