information about use of form

Is possible for me see on the report the address of page where the form is loaded? If is how can do this?

Perhaps the best way is to use the Get Referrer widget. This widget allows you to get the url that directed people to your form.  If neither of the suggestions posted are what you are looking for, let us know and we will be happy to see what else we can do.
