My forms don't open fully on Ipads in Safari

On our website under the forms tab, we have a long form called INTAKE.  When you click it on an Ipad, the form opens but cannot be scrolled to the bottom to finish and click the submit button .  Can we make a form that works fully on Ipads?

You may also like to add "Mobile Responsive" widget in your form which will adjust the width of your form fields without any horizontal scroll.

I replaced the old embed code with the one you suggested.  Its a very long form and with an ipad you are able to scroll down most of the way on the form but the last 7 questions spill over the blue footer on our webpage and cannot be seen.  Other things I can try?
