How do I send a form from myself to other recepients...

Asked by Drafter on September 14, 2011 at 02:32 PM

I'd like to create a form that can be used something like an observation tool.  When management sees something good from someone, I'd like to send them a "nice job" email from an iPad that includes a date/time stamp and who its from along with a few other bits of info.  Is that a possibility?  How?

I am sorry but I got confused by your post. Is this "nice job" email supposed to be automatically sent after the observation tool form is filled out?

Would you care to elaborate more on what exactly your goal is? Thanks!

I'd like to be able to use drop downs and "send" a completed form to employee emails.  I work in education... I'd like to use the form to send observation notes, ideas, etc to teachers whose rooms we visit.  Essentially, we'd need to "send" the form out to email addresses and not receive the form from individuals.

Who exactly is supposed to complete this form? Is it the management or the employees?

Either way, you can probably use conditional notifications to implement this.

First, insert a drop down in your form containing the name of the employees.

Then, create a notification for your form if there isn't any yet.

Afterwards, create conditions corresponding to each employee name, which will send out the notification you created to their designated e-mail address.

    1.  Click Setup & Embed > Conditions
    2.  Select "Send an email after submission" then click Next

    3.  Setup a condition for each employee, like this:

Where Notification 1 is the name of the notification email you created on the first part of this tutorial. jodoe@gmail.com is the (sample) email of Employee "John Doe".

    4.  Replicate this condition for all of the other employees

Click here to learn more about conditions.

By the way, I noticed that you are using a guest account. Kindly register a free JotForm account to prevent losing access to the forms you create and to enjoy up to 100 submissions per month.

Please inform us if you have follow-up questions regarding the steps I provided above. Thanks!
