I was able to see the caused of issue in your jotform http://www.jotform.com/form/1481556697
It was because the Notifier email was probably broken/corrupted due to multiple revisions
Use the Add New Email button in the Email Alerts to add a new Notifier
Kindly inform us if issue persist.
