When setting up Mailchimp the dropdown list is not working.
Please see screenshot.
I do have a 3 list on my mailchimp and working, no issues on my mailchimp.
That is strange. If you have created the "Lists" in your Mailchimp account, they should be listed in integration wizard.
I did clone your form and tried to integrate with my mailchimp account, the list were loaded successfully. I would suggest you to clear your browser cache once and then try to integrate your form again.
