    Hi,

I took a look at your account and I can see one jotform in it called "Title Me". Is this the jotform that you are not able to find?

You also have 2 jotforms in your trash folder. Both of them are called "Clone of Product Order Form".

If you want to restore them, please take a look here on how to Restore Deleted Forms?

Also if you can not find some specific jotform that is neither of the 3 mentioned, please give us the url to it and we will find it for you.

    Best Regards,
    Ben

    Hi Ben,

None of those are the form I'm missing. It should just be titled Order Form. There should also be an Images form that I used as a sort of Library....neither are anywhere to be found...

Like Ben has explained to you, these are the only forms in your account

    Clone of Product Order Form

    Product Order Form

    Title Me

However, let me explain you that both forms are collecting Credit card critical information in plain text fields. This is not allowed and it is also considered a phishing activity, which violates our terms of use

The only services allowed to collect CC information are those that are DSS/PCI certified.

In light of this, it is a must to integrate your forms with any of our Payment integrations.

For now, I've deleted the guilty fields, otherwise, your account might get suspended.
