    Do you have a form where you have started to work with? If yes please share the URL of the form, otherwise, let me share my example:

    Basically you need to add  the Radio button, and then the Text Box button:

    In order to put them on the same line, you need to right click on them and click on the gear icon, then click on "shrink". Please see this guide that to understand form field positioning. Let us know if you have more questions we will be glad to assist you.
