Blank Page on form? Need Help.

    I use page breaks to divide my form into section. On the very first page after I select the required check box and select the next page button move forward but then I get a blank page .

    Is there any possible solution to fix this.

    This is due to field Read and Understand the instructions is shrunk that is displayed as a separate column and effecting page sections. I've expanded this field on your JotForm and it is working fine now.
