Another issue is that I set the condition for the dropdown such that when it is empty, no fields will be shown. However, it didn't appear in the form as I wanted. Could you check where is the mistake I made in setting the condition?

    Hello woolalaa,

I did check your form's condition. It seems you have added condition to only hide the form fields. Please check the screenshot below:

You do not have to necessarily add hide condition. Just add the show condition and if the condition doe not match, the fields will be hidden.

Please take a look at the following cloned form and see if this works as expected:  http://form.jotformpro.com/form/42810267102949? Feel free to clone this form in your account.
