I visited the website url you have posted and was able to see the form.
We had to change the ip address of our dns because of the ddos attack.
It takes time for the dns cache to clear, probably the cache has not been
cleared yet on your ISP level which is why you are getting this issue.
It is close to 24 hours since we updated the ip address and it should
update for everyone soon. Please give it more time,
