Multitype Customer Order Form

Hello!

I am using jotform for my customers to order scones weekly on my website. Is there a way that the form could start off saying are you a returning customer? and if they select no bring them to one form that doesnt ask for address, email, phone number but if they select yes brings them to a form that only asks for their name?

Just don;t want to make my returning customers have to fill all that out every time

Thanks for reaching out to Jotform Support. Based on your description, you should create a Multiple Payment Form. You will have one Main Form that will include the question if the user is a returning customer. Then based on a Show/Hide Condition the other two forms will be presented through iFrame Embed Widgets.

Additionally, in the form that will be created for returning customers, you can include the Spreadsheet to Form Widget and prepopulate the form fields based on the name of the customer.
