Why was account Auto Suspended

    Good Evening. my name is Aliya Griffin and I have a travel agent company. I use Jot Form to create my payment authorization forms. For some reason, the account was just auto suspended. Can someone please verify and let me know what happen. This Jot Form account is crucial to my business. My account email is amazn.travels@gmail.com

    Thank you for reaching out to Jotform Support. We apologize for the inconvenience this suspension has caused you. Upon checking on your account, there's a form that was flagged suspicious.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    Kindly provide us with the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #4417765 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    I do not have a cooperate email address. I was in the middle of booking a trip for a client and the form just deleted. I had to creat a new account under my personal email Amaznliya@yahoo.comI just have one form that I use. It was my companies Payment Authorization form. I uploaded as a PDF and did some edits. Was it considered Phishing because they have to provide credit card details? I really need this form for my clients and Im hoping that the new account does not get suspended. 
    There is not an official URL that I use. Because I am a travel agent, when clients are ready to book their vacation, I use the link on Jotform to send to them and they complete it. I do have a official travel agent website that can be found at the bottom of this email. https://aliyagriffin.inteletravel.com/booktravel.cfm

    Hello Aliya,

    Thanks for reaching out to Jotform Support. Please note that collecting credit card details using regular text fields is not allowed per our Terms. If you need to capture the card details and charge the user at a later date, you will still need to use one of our payment processors and enable payment authorization. Here's a guide that you can check out. We can reinstate your account; however, we will need to delete the offending fields on your form.

    Let us know how you would want to proceed.

    Good morning. Yes, I would love for the account to be reinstated. I will delete the form and use one of the forms provided by Jotform. I will read the guide on how to create the form. Thank you so much.

    I have now removed the offending fields on your form and reinstated your account. Please refrain creating forms that against our Terms of Use.

    I do have a question, can I upload my own PDF form and then use one of the payment authorization processor’s provided by Jotform?
