Box for our use at the end of a form

HI-

I have tried several ways to create a "box" at the end of this form 

After we get the form from the client, we call to make an appt with them- I was trying to set up a box/footer/verbage  at the very bottom that would say 

For WOOF use only:  Interview Day/Date ________________________Tour: Before        After         NA

Everything I try does not work- it's on the form but does not print out 

Help, please!

I inspected the form and could only locate a text area field at the bottom of the form.

I added a Form collapse field . I added a "DateTime" field under Quick Tools menu and a radio-button:

Also for empty fields to show up while printing it is necessary to uncheck an option under submissions' options:
