Form to register and forward phone messages

Rather than hand writing notes to keep track of people who call me and leave me a phone message, I would like a form to use. It would be great if I could then send individual phone messages via email to others that would then be able to reply. It would also be nice if I could save the messages.

Just to clarify "individual phone messages via email"

You mean that when you fill in the form you can send the form to someone by email. if this is correct you may try and clone the demo form created:

With the email notification you will receive a copy of the form, and with the auto-responder, if you fill the email field, the message is also forwarded to someone else.
