The order should if you select New order> Page 2 > submit > Push to email completed form OR select modification > Page 3 > Submit > Push to email completed form

Hi! I have followed the instructions provide but now I am having issues with the page break - the submit button is not working as it just continues onto the medication page

I have fixed the page break issue but we now have a weird button thin going on - I have to have both the page break button that say s next and back AND a Submit button in order to have the third modification page

    Hi wlosdigitalopportunities,

Thanks for getting back to us. Instead of having these on separate pages and skipping to them, which is causing there to be additional pages available and why you're seeing a next button, can you try deleting the page break and having all of them on the same second page. You can then use hide multiple conditions to hide all the field elements that are not associated with the request type that's selected.

Give this a try and let us know if you have any other questions.

    Hello wlosdigitalopportunities,

Thanks for reaching out to Jotform Support. I'm sorry you're having trouble with this. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same problem. After investigating it a bit, I found a solution. It's really easy to fix. Let me show you how:

    I've disabled the conditions below:

Deleted the page 3 and added the submit button on page 2. Additionally, I've added a Section Collapse element before Modify Campaign.

Here is the added condition for New Campaign:

And here is the condition for Modification (I have hidden #7 New Campaign until #103 Submit)

You can check out the cloned form here. You may clone the form to check out the form settings, this guide will show you how to do that.

Give it a try and reach out again if you have any other questions.
