I created a form today and now do not see it in My Forms.

Hi,
Upon checking this form, http://form.jotform.us/form/42876575317163, it seems that it is still live and in your account. If you have accidentally deleted it you can go to the My Forms->Trash and restore it from there. You can also see the history for the revisions made to the form.
Click the restore button to get the deleted form back.

Form Designer shouldn't cause the form to vanish. Actually, the form was created using a guest account. I have just moved it to your account, please kindly check now. The form name is:
Women Achieving Goals for 2015 - Date Selections for Dillon Beach
You can search the form by its ID: 42876575317163.

You are welcome. You will be automatically logged off from the website for a certain time, not exactly sure how many minutes or hours. So make sure that you are logged in when creating a form or when doing modifications to an existing form.
