How to identify the page where Jotform was used

Hi, unfortunatelly I can't get the text instead of "google" I just get numbers in order (1,2,3). Why is it? I would like to identify my pages with a choosen name.

Hi,
We apologize for the mistake on the guide https://www.jotform.com/help/84-How-to-Use-a-Single-Form-in-Multiple-Sites. I just noticed the mistake when you reported here. I am going to correct it later on the user guide.
The 'website' field was NOT suppose to be the Unique ID field as stated
It should be the Hidden Box widget/field instead
Please correct this on your jotform. The steps on should still be the same and the part that is different is the correct widget to use as the hidden field.
