How to disable two columns and go back to one column on a form?

somewhere, somehow I did something that makes my form
display in 2 tables...searched for the settings to put it back...can't find
it...
Do you know what I mean and which section I have to be??

What you need to do is to click on the properties icon near each question and then click on expand. Here is a screenshot:

Is it possible to put just an image at te top of the form (instead of a
slider?)

One issue on my form above....
I can't get rid of the vertical stripe above
my image....
