Please try to use the iframe method to embed your form.
Also I suggest that you add the mobile responsive widget in your form.This will help to make your form mobile responsive.

Hey I added the mobile responsive widget and still not working on the iPhone 5 safari. Haven't tried other phones.

Please try to re-embed your form using the iframe method.
I think that this could be caused by the height set by the default embed method, with the iframe method you can set a custom height which should help to display all the form.

I set scrolling to YES and now it works!!!
