The form was actually disabled because it was suspected as phishing form by the phishing filter.

What I suggest you do instead is create first your JotForm account. I can see at the moment that you are just using Guest account to create your jotform.

You can use this link http://www.jotform.com/signup to create an account for FREE.

Use your email account to signup.

and the jotform you initially mentioned was from the Guest account.

I have fixed every thing for you. 

I have transferred the form to your account and have now reinstated your account.

i can't edit my form and i can't create new form with account fciminiaun

Please tell us the reason why you cannot edit and create new form under your account fciminiaun

Are you not able to login to the account?

I just checked it, and it is an Active account. So, there should be no issue that is account related.

i can login but after login when try edit my existing form it opening but every controls is dimmed so i can't edit it but i can preview it

I just tested it, but I could not reproduced the issue you are reporting. I logged in using your account, and I can Edit and Save the changes on the form.
