How to re-create a specifically designed form?

Need to recreate a form to work through www.jotform.com 

The form has specific font styles and colors and buttons that need to be used. Can we re-create this so we can use through jotform? 

The form that we have created on the website is being blocked somehow by the client's email provider. So we are hoping to use jotform so the email notifications can be received by our client...typically more success of form notifications getting through when using your services.
