I'm being charged double for my subscription

    I'm being double charged for my subscription, but it hasn't upgraded me! My form was shut down due to reaching a limit of the old subscription. But I'm still being charged for both!!!!

     

    Please refund my money for the $9.95 subscription that has been charged after the date of my upgrade, and then continue on with the $19.95 monthly fee!! AND GET MY FORM BACK RUNNING AGAIN.
