My form says 'Form not found' 'This form is disabled'. What do I do?

    My form says,

    This form is disabled.
     
     
    It even shows that on my website.
     
     
    (On the contact and register page)
     
    I need help fast, my website is due on Wednesday.
     
    Any help would be greatly appreciated.

    The problem is that you created it a login form which request username and code/password credentials. This is prohibited by our terms of use
