regarding fill in the blank forms and I injected the custom CSS but it is still not showing up in alignment like the sample form featured here.

This is the link to my form https://secure.jotformz.com/form/42825740285660

    Hi,

I took a look at your jotform and the reason why it is not working for you is that your html elements are much bigger than the ones in the mentioned thread.

The only way to set it up like that would be to add all the text into one Text field and leave the space in approximate size of the fields that should appear in there. We could help you with the CSS code that would place them all in the right space, but I am not sure which section should be changed and which should be left as it is?

I presume that you would like to do this for the fields from top to the address fields then again for the event date and time field?

If you would like to go with this option then please combine the texts into sections with white places or capital letters text describing the position of each field for example: _DATETIME_HERE_ or something similar.

    Best Regards,
    Ben

    Thank you for your response.

I have made the changes to my form:

    http://form.jotformz.com/form/42973852840665

It will include the fields EFFECTIVE DATE through EVENT COST (7 fields total - labeled and in order of their use just below the html text box)

These fields WILL NOT need a label, they are labeled now for the purpose of knowing where they fall within the text.

Please let me know if I have done this correctly. I need this ASAP hopefully by today!!

    Thank you

Since we have multiple lines in between the fields, the text is getting distorted due to some default settings for HTML text. I think you should give a try using our narrative fields widget.

Please take a look at the demo with narrative fields widget and let us know if it works for you.
